Golden Valley ID - New Infrastructure Development Checklist
PURPOSE
This checklist presents jurisdictional and technical requirements and standards of the State of Arizona, Mohave County, and Golden Valley County Improvement District No. 1 (the “District”) for the private design and construction in addition to the conveyance of water system infrastructure elements to the District to own, operate, and maintain while enabling District water system connection and service benefit to the infrastructure improvement sponsor(s). Conveyance of new infrastructure to the District and acceptance by the Mohave County Board of Supervisors, acting as Board of Directors for the District, shall occur in advance of the District permitting any new operational connection to the District’s water system. Multijurisdictional approvals include (1) Arizona Department of Environmental Quality (ADEQ) for public water system design and construction, (2) Mohave County for infrastructure placement within public easements and rights-of-way, and (3) District for infrastructure acceptance.

FEASIBILITY
The following requirements apply to any developer/owner proposing new infrastructure construction for District water system connection:
• Represent property owner within the existing District boundaries.
• Construct new infrastructure element extending parallel to the entire property frontage owned by participating developer/owner.
• Dedicate right-of-way to Mohave County or grant public utility easement, as necessary, for new infrastructure placement.
• Acknowledge and agree to conditions of the Form of Conveyance for Infrastructure.
• Develop infrastructure construction plans and specifications prepared and sealed by an Arizona Registered Engineer. Plans and specifications shall coordinate with standards and guidelines set forth in the District Water Facilities Master Plan Update, latest edition.
• Provide infrastructure construction by an Arizona-Licensed Contractor.
PROJECT APPROVAL / DESIGN
The following requirements apply to the design of water system infrastructure elements for future acceptance and maintenance by the District:
• Call GVID Office at 928-757-0914 to set up a consultation prior to starting the project to ensure waterline can be completed and to determine if there is a Future Development Fee and the cost of the meter installation.
• Submit to the District construction plans and specifications prepared and sealed by an Arizona Registered Engineer. Plans and specifications shall coordinate with standards and guidelines set forth in the District Water Facilities Master Plan Update, latest edition.
• Obtain District approval to construct the project as engineered.
• Upon receiving District approval, obtain ADEQ approval to construct drinking water facilities as required.
CONSTRUCTION
The following requirements apply to construction of water system infrastructure elements for future acceptance and maintenance by the District:
• Use an Arizona-Licensed Contractor to complete project construction.
• Obtain a Mohave County Right-of-Way Use Permit for any work performed within County rights of-way.
• Perform project construction work in accordance with all applicable Maricopa Association of Governments (MAG) Uniform Standard Specifications and Standard Details, Latest Edition, except as otherwise required by the project plans and special provisions.
ACCEPTANCE
The following requirements apply to petitioning water system infrastructure elements for acceptance and maintenance by the District:
• Obtain ADEQ approval of construction for drinking water facilities.
• Obtain approval from governing fire district.
• Submit Certificate of Completion sealed by an Arizona-Registered Engineer accompanied by the following documents:
- As-built plans sealed by an Arizona-Registered Engineer
- ADEQ Water Analysis Report including (1) pressure test results, (2) total coliform test results and (3) chain of custody
- Chlorination test report
- Compaction test data report
- Vicinity map (exhibit on letter-size paper and marked Exhibit “A” for acceptance resolution) Obtain letter of inspection and approval of construction by District Engineer.
• Obtain letter (e-mail correspondence acceptable) from Mohave County Public Works approving release of the Right-of-Way Use Permit for work performed.
• Complete the following forms provided by the District Water Operations Office:
- Form of Conveyance for Infrastructure
- Golden Valley County Improvement District No. 1 agreement for privately constructed water system infrastructure
NOTE: The developer/owner maintains responsibility for infrastructure maintenance until such time the Board of Supervisors accept the petitioned infrastructure elements by resolution.
