The Mohave County Department of Public Health (MCDPH) Vital Records Office processes applications for:

  • Certified birth and death certificates
  • Amendments and corrections to vital records
  • Home birth registrations
  • Special fetal and infant certificates

BIRTH & DEATH CERTIFICATES: QUICK FAQ

Q: CAN I GET MY BIRTH OR DEATH CERTIFICATE THE SAME DAY?
A: No. Birth and death certificates are not issued the same day. Once a complete   
     application and all required documents are received, processing can take up to two   
     weeks.

Q: ISN’T ARIZONA AN OPEN RECORDS STATE?
A: Yes, however birth and death certificates are an exception to that rule.

Q: WHY ARE BIRTH AND DEATH RECORDS NOT OPEN?
A: Arizona law classifies vital records (birth and death certificates) as confidential and limits who can
receive them.

Q: CAN I REQUEST THEM THROUGH PUBLIC RECORDS?
A: No. Public records law does not apply to birth or death certificates.

Q: WHO CAN REQUEST A BIRTH/DEATH CERTIFICATE?
A: Only eligible individuals can request birth & death certificates. 

Q: WHAT IS REQUIRED TO APPLY?

  • Applicant is 18 years of age or older
  • Original completed and signed birth/death certificate application
  • Copy of the front and back of applicant’s valid government-issued picture identification which bears applicants signature or have signature notarized
  • Copies of proof of relationship and supporting documents
  • Correct and exact fee

Drop boxes are available for customer convenience. Applications submitted through a drop box may require additional processing time, as documents must be transported to Kingman for processing. Please ensure all required items are included with your application.

VitalChek provides an online ordering option, but orders placed through VitalChek are handled outside of Mohave County Vital Records. This means county staff are unable to track the application or provide assistance with its status.
