Birth Certificates

BIRTH CERTIFICATES

Mohave County Vital Records can issue certified birth certificates only for births that occurred in Arizona. If the birth occurred in another state, please visit the CDC website for information on how to obtain a copy of the birth certificate.

BEFORE YOU APPLY

Applicants must be 18 years of age or older and meet Arizona’s eligibility requirements  request a certified birth certificate.

Arizona birth certificates are confidential and are not public records. State law limits who can request a certified copy of a birth certificate. Please review the eligibility requirements in (A.A.C.) R9-19-210 and R9-19-211requirements before applying.

*If you submit documents in a language other than English to prove you are eligible for a birth certificate, you must include an English translation. The translator must sign a statement confirming that they are qualified to translate the document and that the English translation is complete and accurate.

APPLICATION FEE & PAYMENT OPTIONS

The state fees for birth records are:

Certified Birth Certificate..................................$20.00

Certified Delayed Birth Certificate....................$20.00

Amendment of Certified Birth Certificate..........$30.00

Mohave County Vital Records accepts the following payment methods for birth certificates:  

  • Cash
  • Money Order – make payable to MCDPH
  • Cashier’s check – make payable to MCDPH
  • Debit/Credit card*

*Debit and credit card are subject to a processing fee. The fee is the greater of $1.50 or 2.39% of the total payment.

APPLYING BY DROP BOX

Each Mohave County Department of Public Health site has a secure Vital Records drop box.

Before submitting your application, make sure you include all of the following:

  • Original completed and signed birth certificate application
  • A copy of the front and back of the applicant's valid government-issued photo ID showing the applicant’s signature, or a notarized signature.
  • Copies of proof of relationship and supporting documents
  • Complete and correct payment amount

Drop boxes are available for customer convenience. Applications submitted through a drop box may require additional processing time, as documents must be transported to Kingman for processing. Please ensure all required items are included with your application.

Monday – Thursday: 8:00 a.m. – 5:00 p.m.

APPLYING BY MAIL

Please note, mail submissions will take longer to process.

Before submitting your application, make sure you include all of the following:

  • Original completed and signed birth certificate application

  • Copies of proof of relationship and supporting documents
  • Complete and correct payment amount
  •  Include a self-addressed, stamped envelope so your certified copies can be mailed back to you.
