Homeschool Registration
Mohave County residents who are parents of and/or a person with legal custody of a child and wish to homeschool their child(ren), must submit a notarized affidavit and proof of the child’s identity with the Mohave County School Superintendent’s office. This affidavit must be filed within 30 days of withdrawing from a public, charter, or private school or upon beginning the homeschooling experience.
The most common proof of identity is a certified copy of a birth certificate, which is a photocopy that has been signed and attested to by a public official as a reproduction of the original document. We also accept the child's baptismal certificate, an application for a social security number or original school registration records when accompanied by an affidavit explaining the inability to provide a certified copy of the birth certificate.
Please do not file a homeschool affidavit if your child is enrolled in a traditional public school, including a virtual charter school (public-school-at-home program) or has received an ESA (Empowerment Scholarship Account) through the Arizona Department of Education.
Register By Mail
- Include a certified copy of the child’s birth certificate or other reliable proof of the child’s identity and age as mentioned above. If you cannot include a certified copy of the birth certificate, include a photocopy and use the space on the affidavit to explain why.
- Print and complete the Affidavit of Intent to Homeschool form, which must be notarized.
- Mail to: Mohave County School Superintendent at PO Box 7000, Kingman, AZ 86402
- We will return your documents by mail within two weeks.
Register In-Person
- Bring a certified copy of the child’s birth certificate or other reliable proof of the child’s identity and age as mentioned above. We will make a copy and return it to you immediately.
- Print and complete the Affidavit of Intent to Homeschool form, which must be notarized. Notary services are available by appointment only at our office and at no additional charge.

- Office hours are Monday – Friday from 8:00 A.M. to 4:30 P.M.
Withdrawing From Homeschooling
The County School Superintendent must be notified within 30 days of the termination of homeschooling using the Homeschool Withdrawal form. If homeschooling is resumed, another Affidavit of Intent to Homeschool form must be filed with the County School Superintendent.

Change of Information
Parents or custodians should use the Homeschool Change of Information form to notify the County School Superintendent of any changes of information.
